Could be anything really that has a story to it or missions. Even pinball if anyone played Space Cadet there was more to it than bashing the ball around the table. Hitting targets did things like upgrade engines or open worm holes etc. a nudist version could be something like hit these targets to win volleyball or hit these targets to chase off building developers (or voyeurs if you wanted to include that element)

I really want to be able to create games with a nudist theme and been stressing out for months now trying to learn but getting nowhere. Only success I’ve had is a simple roll a ball game in unity and only nudist theme you can make from that is switch the ball to a nude person and the enemy object that chases you to a clothed person

What about a mmorpg where all player models are naked ofc, armor never covers to much of your character and if you want to you can turn of vanity to make them completely naked. It would have voice chat and text chat so you can hang out with other naked people and anyone who makes sexual remarks on people can be reported and banned.

There is, or at least used to be, a market for this. There was a blog about nudism in Second Life https://theslnaturist.com/ The blog doesn't seem to be active anymore but I have no idea about the game (or the nude community there).

I did try second life many years ago but aside from it being very laggy on my basic spec laptop I was very aware of some shady stuff going on among certain groups including grooming activity so I stopped using it. Ofc like everything else online it was also full of fakes so it was difficult to judge another users true interest. I might have just been unlucky though and others have had more positive experiences.

I never tried it but mainly because it seemed boring. I think if it came out today it would be a metaverse or web3 product which is even more offputting.

Another one similar to second life is IMVU but your characters don’t move, just do animated poses of designated posing spots in the scene. It was even more boring.
